Suppose you're playing with Scalpelexis and delivering painful damage to an opponent. The card says to exile the top four library cards of that opponent. if two of those cards have the same name - repeat this process.

Do lands count as cards with same names? i.e Mountain-Mountain
Repeat this process how many times? i.e if the next four would be again duplicates would you go on?

Yes. Lands have names, so Mountain, Mountain will result and an extra four cards.
Yes. It doesn't that something like: " This process can be repeated only once" So you can keep milling.

On a side note, I don't think this card is very playable... It's 5CC, Traumatize for example is way better for removal, and has 5CC too.
